NEW YORK, December 15, 2015 — AMC Health, a leading provider of remote patient engagement solutions, has partnered with Advanced Home Care on a multi-year partnership agreement that focuses on a post-discharge telemonitoring program that will help patients with chronic illness comply with their treatment plans and improve their health. AMC Health has provided its real-time virtual care solutions to many home care agencies since its founding in 2002.
AMC Health pairs digital, Bluetooth-enabled devices with a personalized cellular modem in patient’s homes. Devices collect and analyze biometric and behavioral data on a daily basis. Patients occasionally receive interactive voice response (IVR) calls that collect symptom and behavior information to provide a more complete view of their health status. A sophisticated analytics system alerts clinicians to emerging health issues or concerning trends for a specific patient, and delivers meaningful information to the care team’s secure web portal. The patient’s care team uses real-time data to proactively connect the patient with the right intervention at the right time. This includes checking in with patients to educate and guide them on their behaviors, or connecting them to appropriate care when needed.
AMC Health has demonstrated, in peer-reviewed clinical studies, that its patient engagement, health monitoring and care coordination solutions have reduced hospital readmissions for patients with congestive heart failure and reduced their cost of care. AMC Health solutions have also helped patients with diabetes significantly reduce their HbA1c levels and patients with hypertension significantly reduce their blood pressure. These results have been published in the Journal of the American Medical Association (JAMA), Journal of Managed Care Medicine, Medical Care and Population Health Management publications.

With this partnership, AMC Health will deliver the technology, logistics, and healthcare support services for in-home biometric and symptom monitoring Nurses from Advanced Home Care will provide coaching, education, and facilitate intervention with the individual’s physician as needed.
